2024-25 ULTIMATE COLLECTION HOCKEY MAIL-IN PROGRAM
Beginning on March 31, 2026 through December 31, 2026 (“Mail-In Period”) and as a one-time courtesy in connection with The Upper Deck Company (“Upper Deck”) 2024-25 Ultimate Collection Hockey trading card product (“Product”), eligible individuals may submit their Ultimate Chapter Jersey and Patch trading card containing inaccurate authentication copy (each a “Mail-In Card”) to receive a replacement card (“Replacement Card”) and/or replacement products (“Replacement Products”) of equal or greater value, as selected and determined by Upper Deck in its sole discretion. For clarity, the Replacement Card and/or Replacement Products may not be of the same subject as showcased on the Mail-In Card.

HOW TO REDEEM
In order to participate in this program, you must mail to Upper Deck both (a) the Mail-In Card and (b) the completed Ultimate Collection Mail-In Submission Form below (“Form”) during the Mail-In Period to the following address (collectively a “Submission”):

By mailing your Submission to Upper Deck, you acknowledge and agree to Upper Deck’s Privacy Policy found at https://upperdeck.com/privacy-policy/ and these 2024-25 Ultimate Collection Mail-In Program Rules (“Rules”). Upper Deck is not responsible for any lost, stolen, or damaged Submissions, Replacement Cards, and/or Replacement Products while in transit to or from Upper Deck. All Submissions must be received by Upper Deck within the Mail-In Period.
By mailing your Submission to Upper Deck, you represent and warrant that you are the sole legal owner of all rights, title, and interest in and to the Mail-In Card, and you hereby forever release, discharge, and hold harmless Upper Deck and its employees, officers, directors, subsidiaries, affiliates, distributors, agents, successors, and assigns (“Released Parties”) from any and all claims, liabilities, suits, cause of actions, losses, damages, disbursements, costs and expenses (including, without limitation, attorneys’ fees) brought by or on behalf of a third party against any of the Released Parties arising out of, related to, or connected with the Mail-In Card, Submission, Replacement Card, Replacement Products, and Product.
You also understand and agree that all rights under California Civil Code §1542, or any similar international, federal, or state law, are hereby expressly waived. Section 1542 reads as follows:
A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS THAT THE CREDITOR OR RELEASING PARTY DOES NOT KNOW OR SUSPECT TO EXIST IN HIS OR HER FAVOR AT THE TIME OF EXECUTING THE RELEASE AND THAT, IF KNOWN BY HIM OR HER, WOULD HAVE MATERIALLY AFFECTED HIS OR HER SETTLEMENT WITH THE DEBTOR OR RELEASED PARTY.
You understand the importance, meaning, and legal effect of this entire release and your waiver of rights under California Civil Code § 1542, and any other applicable provisions of law. The foregoing waiver further includes without limitation an express waiver, to the full extent permitted by law, by you, of any and all rights under any law of any state or territory of the United States, or principle of common law, which is similar, comparable or equivalent to California Civil Code § 1542.
Once your Submission has been reviewed and validated by Upper Deck, a Replacement Card and/or Replacement Products will then be mailed to you to the mailing address you provided on the Form. Expected delivery of such Replacement Card and/or Replacement Products is on or before December 31, 2026; delivery dates are not guaranteed and may be subject to change. For clarity, Upper Deck will retain possession of the Mail-In Card unless an invalid, incomplete, or otherwise ineligible Submission is provided; in such event, Upper Deck will return the Mail-In Card back to you at the address you provided on the Form. If there is an address change during the Mail-In Period, please send an email, including the new mailing address to Upper Deck at QA@upperdeck.com.
